Credit card payment only. Faxed registrations will not be processed without valid credit card. Allow 3-5 business days for faxed registrations to be processed.

Payment can be made by check or credit card - check must be mailed with registration form. Allow 2 weeks for mail in registrations to be processed.

|SPECIAL NEEDS: To report any special dietary requirements at one of the full-day seminars, please email email@example.com. Be sure to include your name, seminar, city and date.
LAST MINUTE REGISTRATION POLICY: Registrations received within one week of the scheduled seminar cannot be guaranteed handout materials on-site. In this case, course materials will be mailed to the attendee following the seminar.
|CANCELLATION POLICY: Substitutions may be made at any time. Cancellations received 1-3 days prior to the seminar will be charged a $125 service charge. Cancellations and no-shows the day of the seminar will receive no refund. To cancel a registration or make a substitution, email firstname.lastname@example.org or call 800.809.2364.
||HOTEL INFORMATION: Hotel information will be posted 14 days prior to each seminar. Check back frequently for updates. If you are interested in staying at the hotel in which the seminar is located, please call the main hotel phone number and ask to speak with Reservations. AISC has not reserved rooms; therefore you will be quoted room rates based on availability. In the event the hotel does not have rooms available, they will be able to recommend a hotel in the area.|
